The Overnight Hours Nobody Sees in Memory Care

For most, nighttime signals the end of the day, a chance to relax, unwind, and rest. For caregivers in memory care, it's often when their most challenging shift begins.
As daylight fades, many residents living with Alzheimer's disease and other dementias begin experiencing changes in mood, behavior, cognition, and mobility. Residents who were calm during the afternoon may suddenly become anxious, restless, confused, or determined to leave the community. Others may repeatedly get in and out of bed, wander the hallways, or attempt everyday tasks without assistance.
This phenomenon, commonly known as sundowning, is known to affect an estimated 66% of people with dementia and Alzheimer's disease. While researchers are still studying its exact cause, its impact on residents, caregivers, and community operations is well understood.
What Is Sundowning?
Sundowning, or sundown syndrome, refers to a pattern of worsening confusion and behavioral symptoms that typically begin in the late afternoon and continue into the evening or overnight.
It isn't a disease itself, but rather a collection of symptoms that often accompany dementia.
Common signs include:
Increased confusion or disorientation
Restlessness and pacing
Agitation or irritability
Anxiety or fearfulness
Repetitive behaviors
Hallucinations or delusions
Difficulty recognizing familiar surroundings
Attempts to leave the community
Disrupted sleep and frequent nighttime waking
Not every resident experiences sundowning the same way. For some, symptoms are subtle. For others, evenings become the most difficult part of every day.
Why Does Sundowning Happen?
Although no single cause has been identified, researchers believe several factors contribute to sundowning.
Changes in the brain caused by dementia can disrupt the body's circadian rhythm, making it difficult for residents to distinguish day from night. As the day progresses, mental and physical fatigue accumulates, reducing the brain's ability to process information and cope with stimulation.
Environmental changes can make matters worse. As natural light fades, shadows lengthen and familiar spaces become more difficult to interpret. Noise during shift changes, caregiver transitions, fatigue, hunger, dehydration, pain, medication timing, or urinary urgency can all amplify confusion and anxiety.
For someone living with dementia, these seemingly minor changes can create overwhelming uncertainty.
Why Evenings Become High-Risk Hours
Sundowning is more than a behavioral challenge, it is a significant safety concern.
During periods of increased confusion, residents are more likely to:
Stand without assistance
Walk independently despite mobility limitations
Forget to use walkers or wheelchairs
Search for family members
Wander into unfamiliar areas
Attempt to leave the community
Wake repeatedly throughout the night
Make frequent trips to the bathroom
Many of these activities involve transfers like getting out of bed, standing from a chair, or walking to the bathroom, which are among the highest-risk situations for falls in memory care.
For residents living with dementia, overnight hours represent one of the highest-risk periods for falls. In one study of memory care residents, nearly two-thirds (64.6%) of bedroom falls occurred overnight between 8 p.m. and 8 a.m., with incidents peaking between 4 a.m. and 8 a.m.
The Impact on Care Teams
Every memory care professional recognizes the evening routine - one resident begins pacing, another repeatedly asks to "go home," someone else forgets they need assistance before standing, a different resident wakes every hour believing it's morning.
None of these behaviors are unusual. What makes them difficult is that they often happen simultaneously.
Caregivers must continuously prioritize who needs immediate attention while ensuring every resident remains safe, comforted, and respected. This balancing act requires more than remarkable clinical judgment, compassion, and teamwork. It requires continuous visibility into resident movement.
Why Traditional Approaches Aren't Always Enough
Communities have long relied on interventions such as scheduled rounds, bed alarms, motion sensors, and door alarms to help manage overnight risk. These tools remain valuable. However, they are designed to alert caregivers after a resident has already begun a potentially risky activity. They don't reveal that someone has gradually begun making more nighttime bathroom visits over the past week. They don't identify subtle behavioral changes that may indicate an underlying medical issue before a fall or wandering event occurs.
For operators, this creates one of the biggest challenges in dementia care: understanding what is changing before an incident happens.
Understanding the Resident Behind the Behavior
Perhaps the most important lesson about sundowning is that these behaviors are rarely random.
A resident who begins pacing may be anxious. Someone repeatedly trying to leave may be searching for a spouse they believe is waiting at home. Frequent nighttime trips to the bathroom may signal discomfort, pain, dehydration, a urinary tract infection, medication side effects, or another unmet need.
For caregivers, every behavior tells a story. The challenge is recognizing that story early enough to intervene.
Often, the earliest warning signs are subtle: a resident who gets out of bed more frequently than usual, spends longer in the bathroom, begins pacing at a different time each evening, or walks more slowly than they did a week ago. Individually, these changes may seem insignificant. Together, they can reveal that a resident's condition or care needs are beginning to change.
Looking Beyond the Fall
Memory care has made tremendous progress in helping communities respond quickly when an incident occurs. But the next evolution in resident safety isn't simply responding faster, it's recognizing risk sooner.
Every fall begins with a sequence of events. A resident wakes unexpectedly. They sit up in bed. They stand without assistance. They start walking toward the bathroom or into the hallway. Somewhere along that sequence is an opportunity to intervene before the situation becomes an emergency.
The challenge is that many of these moments occur quietly, behind closed doors, and between routine rounds. By the time a caregiver discovers that a resident is on the floor, the opportunity to prevent the fall has already passed.
As senior living continues to embrace more proactive models of care, communities are increasingly looking beyond incident reports and asking a different question: What happened before the fall? Understanding the subtle changes in movement, routines, and nighttime behaviors that precede an incident can help caregivers anticipate changing needs, prioritize interventions, and deliver safer, more personalized care.
